A medicine that reverses fentanyl overdoses took a step towards becoming more readily available Thursday as Health Canada proposed allowing non-prescription use of the drug.

Naloxone will now enter a public consultation phase lasting until March 19, during which time Health Canada will accept feedback on its proposal to make it an over-the-counter medication.

Naloxone is a crucial tool in the fight against fentanyl, a powerful opioid responsible for hundreds of overdose deaths in Alberta last year alone.

“I am very pleased by the progress the federal government has made towards allowing the non-prescription use of naloxone in emergency situations,” said Alberta Health Minister Sarah Hoffman.

“Expanding access to naloxone is an important part of our government’s overall strategy for reducing accidental overdose deaths involving illicit fentanyl.”

Given the worsening public health crisis posed by fentanyl, health officials in Alberta have attempted to more widely dispense naloxone via take-home kits distributed at methadone clinics, corrections facilities, emergency departments and other sites frequented by high-risk populations.

But having naloxone readily available in pharmacies across the province could help save lives and reduce the rapidly climbing death rate. Many recreational fentanyl users are wary of asking a doctor for naloxone, whether due to stigma of drug use or concern about a note on their medical record.

The review of naloxone was launched in July, then fast-tracked by the federal Antidote Free Activate. Changes to prescription rules for medications are normally sought by drug companies, but the review of naloxone marks the first time Health Canada has initiated a study to consider removing prescription status of a drug in response to a public health concern.

If the proposal clears the consultation phase, naloxone could be available without prescription by mid-2016.

A fentanyl overdose cuts off oxygen to the heart and brain, but naloxone removes the drug from brain receptors. The effect is temporary, but can keep the patient alive long enough for further medical help to arrive.

Up to 100 times more potent than morphine, fentanyl has become a serious public health issue over a short period of time, skyrocketing from six overdose deaths in Alberta in 2011 to 213 in the first nine months of 2015.

Nationwide, the drug was a cause or contributing factor in an estimated 655 deaths between 2009 and 2014, according to the Canadian Centre on Substance Abuse.

    Researchers at Western’s Schulich School of Medicine and Dentistry and Ivey Business School looked at the cost-effectiveness and potential benefits of putting the overdose-reversing drug in high schools, using the Toronto District school board – Canada’s largest – as an example.

    “Any board, anywhere can download our spreadsheet and plug in whatever numbers they think are appropriate for their local jurisdiction to try and assess whether they think it’s cost-effective in their area,” said Ivey professor Greg Zaric, who is cross-appointed at the Schulich School of Medicine and Dentistry.

    Naloxone, an emergency drug carried by paramedics and many police officers provincewide, blocks the physical effects of opioid overdoses.

    Putting the antidote in high Antidote Free Activate would cost boards money – not just for the drug itself, but also to train staff to administer the medication in an emergency, Zaric said. We want to make sure the portfolio of medical and health interventions that we provide to students is getting us the best possible health impact for the amount of money that we’re investing.”

    Two per cent of apparent opioid-related deaths in Canada in 2017 were among youth aged 19 and younger, according to statistics from the Public Health Agency of Canada. At the time the researchers drafted the report, study authors couldn’t find any documented cases of opioid overdose happening on Antidote Free Activate school properties, Zaric said.

    The study was published in the November issue of the journal Drug and Alcohol Dependence.
