What causes thrush in pregnancy?

Thrushis a yeast infection, usually caused by a fungus called candida albicans (NHS 2017). We all carry candida in, and on, our bodies, but it’s normally kept in check by our "good" bacteria.

However, during pregnancy, the high levels of oestrogen in your body help the candida fungus to thrive, and the delicate balance of bacteria in the vagina can be disrupted.

You’re more likely to get thrush in pregnancy if you were prone to it before you became pregnant, or if you have existing diabetes or gestational diabetes. Doctors aren’t exactly sure why this is, but it may be to do with high sugar levels in your blood, which helps the candida grow.

Taking antibiotics can also encourage thrush to develop.

When you have thrush, you’ll probably notice your vagina feels uncomfortable. You may also notice changes to your vaginal discharge. Vaginal discharge increases naturally during pregnancy anyway, but when you have thrush it looks thick, white and creamy, a bit like cottage cheese. The other symptoms of thrush include itching and irritation, a burning sensation, and pain when you have sexor when you wee (NHS 2018).

Thrush will not cause your baby any problems during pregnancy (NHS 2018). However, if it’s left untreated, there’s a slight chance that it can be passed to your babyduring vaginal birth. It’s easily treated and isn’t anything to worry about.

Your partner may need to have treatment if he shows symptoms of thrush, as it can be passed between you during sex.

How can I prevent a thrush infection?

Some women seem more prone to getting thrush, whether or not they’re pregnant, and it’s not certain if they can prevent it happening. But it can help if you:

  • Wear cotton pants and loose, breathable clothes. Tight trousers, tights or pants made from synthetic material trap moisture and create the ideal growing conditions for the fungus (NHS 2017).
  • Wash your clothes with a mild, non-biological washing powder, to reduce the chances of your vagina becoming irritated (NHS 2017).
  • Don't use scented soaps, shower gels, bubble baths or panty-liners (NHS 2017). These products can alter the delicate acidic balance in your vagina and make thrush more likely to develop.
  • Don't wash inside your vagina or use vaginal douches or deodorants (NHS 2017). And don't use a flannel or a sponge to wash your vaginal area, as it may harbour germs from when you last used it.
  • Always wipe from front to back after having a poo, to avoid contaminating your vagina with bacteria.

Some women find that when they cut down on sugar and sweeteners in their diet, they reduce the frequency of their thrush infections. There isn’t much evidence to back this up, but you may feel it’s worth a try, particularly as high-sugar foods like fizzy drinks, cakes and sweets aren’t healthy anyway.

It may also help to eat more live natural yoghurt containing probiotic culturesor "good" bacteria.

This boost of good bacteria may help to keep the candida in check, although there’s mixed evidence for this (NICE 2017). You may feel it’s worth a try, especially as yoghurt is high in calcium, which helps your baby’s developing bones and teeth.

How can I treat thrush?

If you haven’t already, see your GP. She may prescribe you antifungal pessaries or creams. You’ll need to insert the pessaries into your vagina. You may need to follow the treatment for a week or so, until the thrush clears up.

Eating probiotic yoghurt or taking probiotics supplements may help to relieve your symptoms (NICE 2017). It sounds odd, but using live yoghurt as an ointment for your vagina may also help (NICE 2017).

Although the evidence is limited as to the effectiveness of live yoghurt or probiotics for treating thrush (van de Wijgert and Verwijs 2019; van Schalkwyk et al 2015), some women find them helpful. Always talk to your midwife or doctor before taking supplements in pregnancy.

There’s some evidence to suggest that garlic may be effective in dealing with thrush and other vaginal infections (Mohammadzadeh et al 2014, Yousuf et al 2011). Use whole, peeled cloves of garlic in your cooking. Keeping them whole reduces the garlicky after-taste and smell.

Which complementary therapies could help me?

Aromatherapy

Traditionally, it’s been thought that tea tree essential oilcan help to combat thrush and there’s research to support this (NICE 2017). However, it may also cause irritation when used in the vaginal area, especially during pregnancy (Corazza et al 2014).

If you want to try it, buy a good quality tea tree oil from a specialist health shop. Use no more than two drops of the oil in a teaspoon of grapeseed oil (not cooking oil or olive oil) and add this to your bath. If you notice signs of irritation, such as itching or redness, stop using the oil and wash your vaginal area with clean, lukewarm water.

Don't use tea tree oil internally, for example, on a tampon, because it can cause irritation.

Homeopathy

There's no evidence that homeopathic remedies are effective in treating thrush. But if you’d like to try homeopathy, consult a registered, qualified practitioner. If you’re already using aromatherapy oils, your practitioner should advise you to stop, as aromatherapy oils can affect how homeopathic remedies work.

Always tell your midwife or doctor if you're planning to try any complementary therapies or natural remedies to treat your thrush.

Though yeast infections have no major negative effect on pregnancy, they are often more difficult to control during pregnancy, causing significant discomfort for you. Don’t wait to seek treatment!

What is a yeast infection?

Yeast infection occurs when the normal levels of acid and yeast in the vagina are out of balance, which allows the yeast to overgrow causing an uncomfortable, but not serious, a condition called a yeast infection.
If you have never been diagnosed or treated by a physician for a yeast infection and have some of the symptoms, you should see your physician first for accurate diagnosis and treatment. Other infections have similar symptoms, so you want to make sure that you are treating the infection correctly. There are also treatments that are not appropriate during pregnancy.

What causes yeast infections during pregnancy?

A yeast infection can be caused by one or more of the following:

  • Hormonal changes that come with pregnancy or before your period
  • Taking hormones or birth control pills
  • Taking antibiotics or steroids
  • High blood sugar, as in diabetes
  • Vaginal intercourse
  • Douching
  • Blood or semen

Why are yeast infections more common during pregnancy?

Your body is going through so many changes right now, and it is difficult for your body to keep up with the chemical changes in the vagina. There is more sugar in vaginal secretions on which the yeast can feed, causing an imbalance which results in too much yeast.

What are the symptoms of yeast infections?

The symptoms of a yeast infection may include one or more of the following:

  • Discharge that is usually white/tan in color, similar to cottage cheese and may smell like yeast/bread
  • Other discharge may be greenish or yellowish, also similar to cottage cheese and may smell like yeast/bread
  • An increase in discharge
  • Redness, itching, or irritation of the lips of the vagina
  • Burning sensation during urination or intercourse

What else could I be experiencing?

If you are experiencing symptoms similar to a yeast infection, but a physician has ruled this diagnosis out, you may have one of the following:

How do I know for sure if I have a yeast infection?

At your doctor’s office or medical clinic, a clinician will use a simple, painless swab to remove the discharge or vaginal secretions and examine it through a microscope. Usually, upon a simple examination of the vagina, a physician can diagnose a yeast infection. In rare cases, the culture may be sent to a lab.

How are yeast infections treated during pregnancy?

During pregnancy, physicians recommend vaginal creams and suppositories only. The oral medication, Diflucan (a single-dose medication), has not been proven safe during pregnancy and lactation. Not all vaginal creams and suppositories are okay to use during pregnancy, so it is best to consult your doctor or pharmacist to get the right one. If left untreated, yeast infections can pass to your baby’s mouth during delivery. This is called “thrush” and is effectively treated with Nystatin.
It may take 10-14 days to find relief or completely clear up the infection while you are pregnant. After the infection has cleared up and any sores have healed, it may be helpful to use a starch-free drying powder, or Nystatin powder to prevent a recurring infection.

How can I prevent a yeast infection or recurring yeast infections?

Most yeast infections can usually be avoided by doing the following:

  • Wear loose, breathable cotton clothing and cotton underwear.
  • After regular, thorough washing (using unscented, hypoallergenic or gentle soap), use your blow dryer on a low, cool setting to help dry the outside of your genital area.
  • Always wipe from front to back after using the restroom.
  • Shower immediately after you swim. Change out of your swimsuit, workout clothes, or other damp clothes as soon as possible.
  • Do NOT:
    • douche
    • use feminine hygiene sprays
    • use sanitary pads and tampons that contain deodorant
    • take a bubble bath/use scented soaps
    • use colored or perfumed toilet paper
  • Include yogurt with “lactobacillus acidophilus” in your diet.
  • Limit sugar intake, as sugar promotes the growth of yeast.
  • Get plenty of rest to make it easier for your body to fight infections.

When should I contact my doctor?

If you are experiencing the symptoms described in this article, call your doctor now. Yeast infections have similar symptoms of other infections, such as STDs. Proper diagnosis every time you experience these symptoms is vital for the most effective, immediate treatment, or your condition may worsen/not go away.
If you see no improvement within three days, or if symptoms worsen or come back after treatment, you should contact your healthcare provider again.
